Veterans Community Nursing Services
Community Nursing Services help eligible DVA clients receive clinical care in the comfort of their own home, with health care and personal care services delivered by our caring and supportive team.
Our nursing team can care for wounds, help with medication and assist in managing the symptoms of medical conditions. We provide support after an operation or stay in hospital, monitor overall health, and offer compassionate end-of-life and palliative care.
Where there is a clinical need, we can also assist with day-to-day activities such as showering, toileting and dressing – care that preserves dignity while meeting genuine need.
All veterans, war widows and widowers holding a Veteran Gold Card can receive DVA-funded Community Nursing services. Some Veteran White Card holders may also be eligible where they meet the criteria.
To get started, speak with your local doctor or GP, a nurse practitioner in community services, a doctor at the hospital where you are receiving treatment, or the person managing your hospital discharge.
They will organise a referral to a DVA-contracted Community Nursing provider, who will assess your care needs – then our team takes it from there.
